The Registered Nurse (R.N.) Supervisor's Duties:
The registered nurse supervisors are responsible for supervision of services such as nursing services,
personal care services, companion sitting and all services provided to our clients. Each of our health care
clients receives an initial assessment from a registered nurse to determine his/her particular needs, desires
and limitations. The RN will review the physical fitness of the client, examine their medications, consult
their physician, conduct home visits for client evaluation, employee training, review care environment to
create and maintain safe and functional atmosphere.
The R.N. supervisor shall assess the client's needs and establish a written plan or service in collaboration
with the client and/or responsible party.
Conduct annual staff observation or demonstration of job tasks, and ongoing monitors through in-home
visits with clients.
Updated care plans for Nursing Services every sixty- two days or as needed, Personal Care every ninety
two-days, and Companion Sitting every one hundred twenty-two days or when provider receives a serious
complaint.
Scope ofthe supervisory visits is the assessment of the client's general condition, progress, problems,
satisfaction and observations about levels of service being offered.
The Registered Nurse (RN) has the following supervisory functions:
• Meet with the client and, when appropriate, the client representative and/or caregiver at least every
two months (every 60-62 calendar days) to conduct a face to face supervisory visit with the client
whose health status and situation involve complex observations, and to complete initial assessment
of client
• Conducting in home supervisory visit at least once per year to observe and monitor the in home
performance ofthe personal support aides who provide care to those clients whose health status
and situation involve complex observations.
• Completing timely, pertinent supervisory notes that include the documentation required.
• be familiar with True Companions, Inc. Policies And Procedures for Care Services
• maintain working knowledge of Rules and Regulations for Private Home Care Providers
• review referrals from CCSP, HMO's, PPO's, coordinators, and other providers evaluating the
clients needs during the initial and subsequent visits, and identifying and assigning the appropriate
staff to provide the client care needed
• develop and revising the client care plans at least every two months (60-62 calendar days)
• supervising personal support aides and license practical nurses
• complete clinical records, including documenting progress notes and supervisory visit entries
• reviewing, signing, and dating all clinical record entries (i.e., service record forms, progress notes,
supervisory visits) made by the personal support aides and license practical nurses
• Communicating client care needs with other providers, clients, client representatives, caregivers,
and/or care coordinators
• making appropriate recommendations to provider, and care coordinators regarding client services
• conducting face-to-face visits at least every two months (every 60-62 calendar days) to re-evaluate
the needs ofthose clients whose health status and situation involve complex observations
• arranging for orientation and in-service training of staff, supervises the 40 hours of training
required, document staff and aides files to include orientation, objectives, content, time spent per
subject matter, instructor(s), and evaluation
• educating client, client representative, and/or caregiver(s) about the client health status, level of
functioning, and health care needs.
License Practical Nurse (LPN) Duties:
Qualifications and duties of the (LPN) include:
• A current license to practice nursing and render services as a LPN in accordance with the Georgia
Practical Nurse Act, O.C.G.A. 42 -26-30 et seq
• A minimum of two years experience in the home health services or related field; prefer experience
working with older and disabled populations
• Ability to supervise certified nurses aides, Adult Health assistants, and personal support staff, have
good written and communication skills
• Working knowledge of the rules and regulations for Private Homecare Providers
• Monitoring and recording client's vital signs as needed
• Observing client's functional level; reporting any changes to the client's physical condition and
health care needs to supervising (RN)
• Assisting clients, as needed, with medications, observing for possible reactions; reporting
reactions to the supervising (RN)
• Preparing progress notes and supervisory visit entries; documenting client's current status
• Teaching self-care activities to clients, client representatives, and caregivers
• Assisting with and/or providing client care as needed
• Collaborating with the supervising (RN) to update their client care plans
• Supervisory functions of the (LPN) include: Completing a face-to- face supervisory visit with
assigned clients and, when appropriate, the client representative and lor caregiver, at least every
two months (every 60-62 calendar days); notifying the supervising (RN) of any significant
changes in the client's status
• Completing an in-home supervisory visit with assigned clients at least once per year to observe
and monitor the in-home performance of the personal support aides
• Completing timely, pertinent supervisory notes.
• The administration of treatments and medications. LPN's prepare and record clinical notes and
maintain ongoing contact with the RN who initially assessed the client. They report any changes
or causes of concern in the client's condition to the RN.
Certified Nurses Assistant's (CNA's) and Personal Support Aide (PSA's) Duties:
Is to provide to all client's to include elderly handicapped and convalescing individuals; transport and
escort services; meal preparation and serving; and household tasks essential to cleanliness and safety.
Housekeeping includes sanitation; home safety; handling medical emergencies in the home; and
infection control. Hands on service provide bathing, toileting, assistance with ambulation and transfer
when assigned to a client.
The Aide must maintain the following duties and responsibilities when caring for a client:
• Safety and accident prevention, fire safety
• budgetinglhome economics
• effective communication with older adults
• caring for client with Alzheimer's and other related disorders
• observation skills; reporting and documentation procedures
• Possess a caring and understanding attitude toward individuals, who are elderly, frail, or disabled
• be flexible and tolerant of varied life styles
• be able to work under supervision and within the guidelines of a care plan
• be in good health
• Home Companion sitters must complete orientation and training prior to working with our clients
or in their households.
• CNA's and companion sitters are not nurses and are not permitted to administer medications.
They are permitted to monitor blood pressure, temperatures, and food intake, assist in selfmedication,
and reporting all their findings to the RN.
Responsibilities:
Provide or assist with any of the appropriate duties assigned, and adhere to written code of ethics and dress

Encourage client to make decisions and remain as independent as possible, and apply information acquired

Job Description:
Work from order sheets to build loads for transport, bulk and/or route trucks, using a forklift and/or electronic pallet jack. Each type of loading requires strict attention to order accuracy, both on what is loaded onto outgoing trucks and what is unloaded from incoming trucks. Transport truck loading involves moving full pallets of product from the warehouse onto a trailer, and removing any returned pallets/plastic flats. Include unloading of raw materials and placing them in designated warehouse space. Bulk truck loading involves selecting designated packages and flavors of product from the warehouse, then loading them onto a pallet. Responsible for maintaining the stability of the pallet by properly stacking and shrink wrapping the built pallet. In most cases, load pallets onto a trailer for local delivery. Unloading returned product and pallets/plastic flats. Conventional truck loading involves selecting designated packages and flavors of product from the warehouse, then loading them into the bays of a route truck which the salesperson selected for that particular product/package - includes unloading returned product/pallets/plastic flats. This is a very physical position. Position requires lifting product weighing from 20 - 45 pounds repeatedly during a 10-12 hour work period. Requires constant standing, walking, bending, twisting, squatting reaching and grasping as you move product
PRIMARY JOB ACCOUNTABILITIES:
